Home Health care is a wide range of health care services that can be given in your home. It is usually less expensive, more convenient, and just as effective as care you can get in a hospital or Nursing facility.
The goal of Home Health care is to treat an illness or injury. Home Health care helps you get better, regain your independence, and become as self-sufficient as possible. Home Health and Private Duty Care are very similar in care and procedures but the requirements to qualify as a Home Health patient differ.
Home Health care is often covered, at least in part, by Medicare, Medicaid, or Private Insurance. Your Doctor must order the care for you. One of the requirements is for the patient to be Homebound.
To be Homebound means:
- Leaving your home is not recommended because of your condition
- Your condition keeps you from leaving home without help (such as using a wheelchair or walker, needing special transportation, or getting help from another person)
- Leaving home takes a considerable and taxing effort
- You may leave home for medical treatments or short, infrequent absences for non-medical reasons, such as attending religious services.
- You can still get Home Health care if you attend adult day care, but the care services would take place in your home
In general, Home Health care includes part-time or intermittent skilled nursing care and other skilled care services such as physical therapy, occupational therapy, and speech-language pathology therapy. We work with your physician and their staff to understand and document their preferred care path. Standing orders from each physician are prepared ahead of time and ready to be authorized at the time of hospital release.
